If you live near our practice in Jacksonville Beach Florida, we’d love to see you in person. But don’t feel constrained by geography. Even if you live a thousand miles away—and many of our patients do—you can schedule a thorough and deeply informative phone consultation with Dr. Davenport.
New Patient “Jump Start” Consult Office or Phone Consult. Consults typically are 30-45 minutes.
Due to limited spaces available each month, we can only take a select number of applicants. If interested, please get in touch with us ASAP.
Here’s how it works: Email us with your name, address, and phone number. One of our staff members will contact you to schedule your appointment and answer any questions you may have.
Click the link at the bottom of this page to get our new patient questionnaire. These forms – which include a detailed history, adrenal, yeast, metabolic, thyroid, brain chemistry, and nutritional questionnaire—must be received by us before your scheduled consult.
